Apply for Lifetime Membership
Applying for membership is easy! Simply complete an application form online (see below) or fill out and email the form to applications@chp11-99.org. Pay the one-time tax deductible fee for the corresponding membership level you choose and submit payment with your form.

Process & Timing
Your application will be reviewed and a background investigation will be conducted in approximately 5 to 10 business days from when we receive your information/application. If your application is approved, any items you selected on your application to receive will be ordered at that time. Please allow at least 4-6 weeks from approval for delivery of membership items. All information you provide on your application is kept strictly confidential and secure.
Potential Reasons for Denial
If you have multiple recent citations on your driving record, you may be denied for membership. If you have any felonies on your record, including a DUI, you will not be approved for membership.

Paying By Check?
If you wish to pay your membership fee by check, please fill out the PDF Application Form and send it in with your check.
Tax Acknowledgements
Your tax acknowledgement letter will be sent within 3-4 weeks following your membership approval. Should you require specific wording or the letter be sent to a different addressee than the individual member (e.g. If paying through a donor advised fund), please email info@chp11-99.org with your specifications.
Note: The Fair Market Value (FMV) of any items received with your membership is not tax-deductible. View item photos and FMVs here.
