To place an order, please email your completed form to store@chp11-99.org by 11:59 pm on 7/5. Orders will be fulfilled in the order they are received. Only existing 11-99 members qualify. Member ID must match member name on order form. A FREE tote bag will be given to the first 25 members who place an order.
