With Less Than 24 Hours Left, Follow These Steps to Enter:
1. Call our office 714.529.1199 to speak with a staff member & provide entry information + credit card payment over the phone, OR

3. Fill out the entry form with your information + payment. Only 2,500 tickets are being sold. TIX = $100 EACH.
4. Save the PDF form on your computer/device (Mac users may have issues with the fillable fields if the PDF is downloaded before being filled out)
5. Email/scan your form back to info@chp11-99.org for processing and your ticket stubs will be scanned and emailed back to you.
ALL FORMS + PAYMENT must be received by 5 PM on 10/7 to qualify. The random drawing is on Thursday, October 8. Winners will be announced via email, on Facebook, Instagram, and posted on our website on the 8th! Winners will be notified by phone, mail, and/or email.
